Donors make a difference by impacting the work and mission of Transitions LifeCare with their support.

The Legacy Society recognizes donors who have included Transitions LifeCare in their estate plans. We are deeply honored by the generosity of those who choose to make a lasting contribution and a real difference in the lives of Transitions LifeCare patients, their families, and the community. Your gift will help us to provide hospice and palliative care to everyone – children and adults - regardless of their ability to pay, and bereavement services to their families.

Bequest in your will benefitting our organization
Naming Transitions LifeCare as a beneficiary of a life insurance policy, IRA or other investment
Charitable Remainder Trust, Unitrust, Annuity Trust or a Living Trust
